Have a serious injury or illness? Carolina Care Plan offers our Case Management Program to help ensure you have access to the right care, at the right time and at the right cost.
Our comprehensive program, which provides assistance for about 50 conditions, works with you and your doctor to develop and implement an individualized plan that promotes safe and cost-effective care.
This voluntary program is offered to eligible members at no out-of-pocket costs. Read the brochure for more detail.
Philosophy and Ethical Framework for Practicing Case Management
Ethical principles are relevant and meaningful concepts that clarify or guide Case Management practices. Our Case Management department practices within established ethical guidelines.
Case Management serves as a means for achieving member wellness and autonomy through advocacy, communication, education, identification of community resources and service facilitation. The Case Management philosophy is based on the recommendation that individuals, particularly those experiencing catastrophic injuries or chronic illnesses, be evaluated for Case Management services.
Case Management principles, interventions and strategies are targeted toward the achievement of member stability, wellness and autonomy. They are based on the needs and values of the member and are pursued in collaboration with the healthcare team.
