A network is a group of hospitals, clinics, medical centers and physicians that participate with Carolina Care Plan and its affiliates. Prior to obtaining services, you can ensure maximum cost savings and avoid unexpected charges by verifying the status of providers.

If you are a current member of Carolina Care Plan, please select the logo that matches the one on your ID card. This will ensure you are selecting the appropriate network for provider participation in the designated service area.
